Because from the outside, everything looks fine:
- You’re competent
- You’re respected
- You’re doing meaningful work
But internally?
Something has shifted.
And you don’t quite have the language for it.

This Isn’t Burnout (At Least Not Yet)
Burnout is often the label we reach for.
We tell ourselves:
- “I just need a break”
- “I need a holiday”
- “I need better boundaries”
And sometimes that’s true.
But sometimes…
👉 the real issue isn’t exhaustion
👉 it’s misalignment

What Is Professional Identity — And Why Does It Matter?
Your professional identity is how you understand yourself in your work.
It’s not just your job title.
It’s:
- how you see your role
- what you believe your contribution is
- how you define your value
And here’s the important part:
👉 your professional identity evolves over time
But your role doesn’t always evolve with you.

When Identity and Role No Longer Match
In psychology, there’s a concept called identity dissonance.
It’s when:
who you are no longer matches what’s being asked of you
In organisational psychology, we also talk about role conflict —
when different parts of your role require different versions of you.
This is common in healthcare, especially when you move from:
- clinician → manager
- clinician → business owner
- clinician → leader

A Story You Might Recognise
I worked with a clinician who built a multi-clinician practice during the pandemic.
From the outside, it looked like success.
- A growing team
- Strong demand
- Business systems in place
But behind the scenes?
She was:
- waking up at night crying
- working hours just to keep things afloat
- taking loans to make payroll
- hoping staff wouldn’t resign
And when they did?
Everything felt like it might collapse.

At one point, she said to me:
“I miss being good at something.”
She had been a highly skilled clinician.
She helped people.
She made a difference.
But now?
She was managing people, managing risk, managing systems —
and barely doing the work that originally mattered to her.

Then came the question:
“Is it OK… to not want this anymore?”

This Wasn’t Failure — It Was Identity Dissonance
She thought she had failed.
That she couldn’t handle the pressure.
But that wasn’t it.
👉 Her identity had outgrown the role she had created
She had built a business that required her to be someone
she no longer wanted to be.

The Moment Everything Shifted
What changed wasn’t the external environment overnight.
What changed was this:
She allowed herself to say, “I want to be good at something again.”

She moved back into:
- 1:1 work
- consulting
- speaking
Not smaller.
👉 Just more aligned.

A Hard Truth for Health Professionals
Some of you reading this don’t need to grow your business.
You don’t need:
- more staff
- more referrals
- more complexity
👉 You need to return to the part of your work that actually fits who you are now.

Why This Matters for Capacity
This links directly to capacity — and this season’s theme: Capacity, Not Cost.
When your identity and your role are aligned:
- your work feels sustainable
- your energy stabilises
- your decision-making improves
When they’re not?
- everything feels heavier
- your capacity drops
- your tolerance shrinks
Not because you’re incapable…
👉 but because you’re misaligned.

The System Doesn’t Help
Healthcare systems reward:
- growth
- efficiency
- output
Not necessarily:
- alignment
- sustainability
- long-term outcomes
So it’s easy to believe that:
👉 bigger = better
But that’s not always true.

The Real Risk of Ignoring This
If you ignore identity misalignment long enough:
- it can lead to burnout
- it can lead to disengagement
- it can lead to leaving the profession entirely
And often…
👉 we lose highly capable clinicians
not because they can’t do the work
but because they can’t keep being who the work requires them to be

What You Actually Need (It’s Not More Education)
Most clinicians don’t need more courses.
They need:
- high-quality supervision
- meaningful consultation
- space to think and integrate
Because the issue isn’t lack of knowledge.
👉 It’s lack of alignment.

If This Is You…
If something in your work no longer fits…
You don’t need to fix it today.
But you might need to:
- stop dismissing the feeling
- stop labelling it as “just burnout”
- start getting curious about what has changed

A Final Thought
You can be successful in your career…
…and still outgrow the version of yourself that got you there.
And when that happens?
It doesn’t mean something is wrong with you.
